Job summary

Martha Franks Retirement Community in Laurens, SC seeks an experienced MDS Coordinator (RN/LPN) to manage MDS 3.0, RAI, and PDPM processes for our skilled nursing and rehabilitation center.

You will lead interdisciplinary MDS/care plan meetings, ensure regulatory compliance and documentation accuracy, and support staff education and quality improvement initiatives within a supportive Oakdale Seniors Alliance environment. This is a full-time role with competitive compensation and benefits.

Qualifications

• Oversee MDS 3.0 assessments and RAI processes per CMS timelines.
• Ensure PDPM reimbursement alignment with resident needs and services.

Responsibilities

• Oversee scheduling, completion, and electronic transmission of MDS assessments and care plans.
• Lead weekly MDS/Care Plan meetings with interdisciplinary teams.
• Ensure documentation reflects acuity levels and services for PDPM/Medicare-M Medicaid.
• Monitor regulatory changes and participate in QAPI initiatives.
• Create and maintain comprehensive resident care plans based on assessments.
• Provide education on documentation requirements and MDS updates.
