Job summary

Martha Franks Retirement Community is seeking an experienced MDS Coordinator (FT) to manage MDS 3.0 and RAI processes for our skilled nursing and rehabilitation center. You will lead interdisciplinary efforts to ensure accurate assessments and compliant care planning.

In this role, you will coordinate, complete, and submit MDS assessments within regulatory timelines, collaborating with nursing, therapy, and ancillary teams to deliver high‑quality, person‑centered care and documentation aligned

Qualifications

• Active SC RN or LPN license (RN preferred).
• 1–2 years of MDS Coordinator experience in SNF/long‑term care.
• Thorough knowledge of MDS 3.0, RAI, PDPM, and Medicare/Medicaid regulations.
• RAC-CT certification desirable or willingness to obtain.

Responsibilities

• Oversee scheduling, completion, and electronic transmission of MDS assessments, CAA reports, and care plans within CMS/State timelines.
• Lead weekly MDS/Care Plan meetings with nursing, therapy, dietary, social services, and activities teams.
• Ensure documentation reflects resident needs and services for PDPM reimbursement.
• Monitor regulatory changes and participate in QAPI initiatives.
• Create and maintain comprehensive, individualized care plans.
• Educate staff on documentation requirements and MDS updates.
