Qualifications

• Current Colorado RN or LPN license in good standing.
• Current CPR Certification.
• MDS experience preferred.
• Long-term care experience preferred.

Responsibilities

• Coordinate and complete the MDS assessment process in accordance with state and federal regulations.
• Develop and maintain individualized resident care plans with the interdisciplinary team.
• Ensure all MDS documentation is accurate, complete, and submitted on time.
• Monitor assessment schedules and regulatory deadlines.
• Review clinical documentation to support quality outcomes and reimbursement.
• Collaborate with nursing, therapy, dietary, social services, and other departments.
• Support survey readiness and ongoing regulatory compliance.
• Maintain resident-centered care standards and facility policies.
