Qualifications

Responsibilities

• Coordinate the MDS assessment process for assigned residents
• Complete MDS assessments accurately and within required timeframes
• Review charts and documentation for accuracy and completeness
• Develop and maintain comprehensive, person-centered care plans
• Monitor resident status and communicate changes to the care team
• Support quality assurance and survey readiness
