Job summary

Apploi in Illinois is hiring an MDS/Care Plan Coordinator to ensure resident assessments and care plans are accurate, up-to-date, and aligned with the care provided. This role communicates resident conditions to families and coordinates care plans across nursing and therapy teams.

You will oversee MDS assessments, lead meetings, train staff on EMR documentation, ensure regulatory compliance, and maintain confidentiality while collaborating with the Director of Nursing and interdisciplinary team.

Qualifications

• Current Illinois LPN/RN license required.
• AANAC certification preferred.
• MDS experience required.
• Strong organizational, planning, and managerial skills.
• Working knowledge of nursing services, administration, rehabilitation, and EMR systems.
• Ability to initiate, complete, and update care plans.
• Experience training staff on care plans, documentation, and EMR use.
• Knowledge of JCAHO, OBRA, IDPH, and HFS documentation standards.

Responsibilities

• Oversee completion of MDS assessments at admission, readmission, quarterly, annually, and PPS periods.
• Develop and maintain a monthly MDS assessment and Care Plan Conference schedule.
• Ensure timely completion of MDS sections and validate accuracy.
• Collaborate with departments to identify and resolve MDS-related issues.
• Review diagnosis coding with physicians quarterly and update ICD-9/ICD-10 as necessary.
• Lead weekly MDS Pre-Planning and Medicare meetings to ensure compliance.
• Assist and educate staff in EMR charting, documentation, and achieving Care Plan goals.
• Communicate care plans and progress to staff, residents, and families.
• Audit monthly EMR charting and follow up on discrepancies.
• Coordinate resident Care Plans with therapy departments.
• Ensure nutritional assessments are completed and documented with dietary input.
• Maintain resident confidentiality and adhere to safety and infection control policies.
