Job summary

Regalcare in the Wakefield, MA area is seeking an experienced MDS Coordinator (RN/LPN) to join our full-time team. You will ensure timely completion of MDS assessments, accurate coding, and compliance with state and federal regulations.

The ideal candidate holds a Massachusetts RN or LPN license, with at least 1 year of MDS experience in a skilled nursing facility. Strong knowledge of RAI, PDPM, and Medicare/Medicaid guidelines is required.

Qualifications

• Must have strong attention to detail for accurate MDS coding and documentation.
• Excellent communication and organizational skills for interdisciplinary collaboration.
• Ability to work independently within a team-based care setting.

Responsibilities

• Complete and coordinate MDS assessments per state and federal guidelines.
• Ensure accurate coding and documentation to support reimbursement and quality measures.
• Collaborate with interdisciplinary team to develop and update care plans.
• Monitor resident progress and ensure regulatory compliance.
• Participate in care plan meetings and quality assurance programs.
