RegalCare at Greenfield is seeking an experienced MDS Coordinator to join our skilled nursing and rehabilitation center. The ideal candidate is a licensed nurse who can ensure accurate and timely completion of MDS assessments while complying with state and federal regulations.

Qualifications

• Current MA RN or LPN license required.
• Minimum 1 year MDS experience in a skilled nursing facility.
• Strong knowledge of RAI process, PDPM, and Medicare/Medicaid guidelines.

Responsibilities

• Complete and coordinate MDS assessments in accordance with state and federal guidelines.
• Ensure accurate coding and documentation to support reimbursement and quality measures.
• Collaborate with interdisciplinary team to develop and update care plans.
• Monitor resident progress and maintain regulatory compliance.
• Participate in care plan meetings and quality assurance programs.

$8,000 Sign On Bonus! A well-established skilled nursing and rehabilitation center in the Wakefield MA area is seeking an experienced and detail-oriented MDS Coordinator to join our team full-time. The ideal candidate will be a licensed nurse with strong clinical assessment and documentation skills who can ensure accurate and timely completion of MDS assessments and compliance with all state and federal regulations.

Responsibilities:

• Complete and coordinate MDS assessments in accordance with state and federal guidelines.
• Ensure accurate coding and documentation to support reimbursement and quality measures.
• Collaborate with interdisciplinary team members to develop and update care plans.
