Job summary

Woburn Rehabilitation and Nursing Center seeks a dedicated MDS Coordinator with PPS experience to join its healthcare team in Woburn, MA. You will coordinate resident assessments, develop care plans, and ensure high-quality, resident-centered care while maintaining regulatory compliance.

The role requires an RN or LPN with MDS and PPS experience, strong assessment and documentation skills, and ability to work with an interdisciplinary team.

Qualifications

• Current, unencumbered Massachusetts RN or LPN license.
• Minimum of two years of MDS and PPS experience.
• Previous care planning experience required.
• Strong assessment, documentation, and organizational skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively with an interdisciplinary team.
• Strong knowledge of MDS, PPS, and regulatory requirements.
• Commitment to providing high-quality, resident-centered care.

Responsibilities

• Conduct and coordinate development and completion of comprehensive resident assessments.
• Coordinate and support development and implementation of individualized resident care plans.
• Collaborate with interdis dolinary team to develop assessments and care plans.
• Ensure MDS-related documentation is accurate and submitted within timelines.
• Participate in facility survey readiness and throughout the survey process.
• Communicate with nursing staff, physicians, therapists, social workers, and other team members.
