Job summary

Asbury Solomons, part of Asbury Communities, Inc., seeks a Lead in MDS Coordination to manage PPS/MDS scheduling and interdisciplinary care planning. The role supervises nursing staff, coordinates assessments, and drives accurate MDS completion in a waterfront 40-bed facility in Maryland.

The position requires MD RN licensure and 2 years long-term care experience, with RAC-CT and PointClickCare familiarity preferred.

Qualifications

• Must be a Registered Nurse currently licensed in the State of Maryland.
• Two years of long-term care experience.
• RAC-CT preferred but not required.
• Experience with PointClickCare preferred but not required.
• Ability to follow and give accurate written and oral instructions; strong reading of reports.

Responsibilities

• Conceives and creates care plans based on a comprehensive resident assessment through interviews, physical assessment, observation, and input from the care team.
• Schedules PPS/MDS assessments within the established time frame for residents admitted to the nursing care center.
• Leads the assessment coordination process to ensure accurate and timely completion of all nursing sections of MDS assessments.
• Compiles data for resident insurance updates for PPO and managed care payer sources.
• Communicates effectively with D.T. Therapy and Insurance Case Managers to maximize RUG levels for reimbursement.
• Leads the MDS completion process ensuring accuracy and timeliness across disciplines.
• Works with the interdisciplinary team to coordinate care plans/assessments.
• Supervises nursing staff as needed and participates in Nurse Management on-call rotation.
