Job summary

Seneca Place is seeking an MDS Coordinator (RN) to supervise MDS and care plans for residents, ensuring accurate documentation and compliance with state rules. The role requires current RN licensure and LTC familiarity, plus data entry and MDS 3.0 knowledge.

Responsibilities include coordinating care plans, securing timely submissions, and leading care plan meetings with the care team to maintain quality resident care.

Qualifications

• RN license active and in good standing in the state of employment.
• Experience in Long Term Care preferred.
• Data entry and word processing skills.
• Knowledge of MDS 3.0 and related guidelines.
• Understanding of laws and regulations for skilled nursing facilities.
• Ability to plan, organize, develop and interpret care programs; CPR certification within 30 days.

Responsibilities

• Coordinate all MDS and care plans per facility policy.
• Inform care team when care plans are due and ensure component completion.
• Develop or revise resident care plans quarterly or with condition changes.
• Submit MDS, CATs, and CAAs to state/federal agencies per guidelines.
• Schedule and lead care plan meetings with the team.
