Job summary

Andover Forest Post Acute Care Center is seeking a full-time MDS Coordinator RN in Massachusetts to oversee the MDS process, coordinate comprehensive assessments, and ensure accurate clinical documentation for proper reimbursement.

You will collaborate with nursing, therapy, social services, and other members of the interdisciplinary team to ensure resident needs are accurately assessed and documented. Active MA RN license and MDS experience are required.

Qualifications

• Active Massachusetts RN license required.
• MDS experience in skilled nursing or long-term care.
• Strong knowledge of MDS assessments, care planning, and reimbursement.
• Excellent organizational and communication skills.
• Direct hire position – not an agency.
