Job summary

Regalcare of Holyoke is seeking an experienced MDS Coordinator to join our leadership team in a full-time role. The position focuses on accurate MDS assessments, RAI management, and interdisciplinary care planning within a resident-centered setting.

In this role you will collaborate with nursing, therapy and administrative teams to ensure compliant documentation, supporting outcomes and reimbursement. Strong organizational skills and independent deadline management are essential.

Qualifications

• Active RN or LPN license in good standing.
• Prior MDS experience in a long-term care setting preferred.
• Strong knowledge of Medicare/Medicaid guidelines and RAI process.
• Excellent organizational and time-management skills.
• Ability to work independently and meet deadlines.

Responsibilities

• Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations
• Manage the RAI process and support interdisciplinary care planning
• Ensure documentation supports clinical outcomes and reimbursement
• Collaborate with nursing, therapy, and administrative teams
• Monitor assessment schedules, audits, and quality measures
• Support surveys and quality improvement initiatives
