Seneca Place in Pittsburgh, PA seeks an experienced RN to join as an MDS Coordinator. You will conduct comprehensive resident assessments, develop and update care plans, and ensure timely documentation and regulatory compliance.

Qualifications

• RN license required.
• Minimum 2 years in long-term care and MDS coordination.
• Strong communication and interpersonal skills.
• Excellent organizational and time management skills.
• Ability to work independently and collaboratively in a team.
• In-depth knowledge of state and federal long-term care regulations.

Responsibilities

• Coordinate and complete comprehensive resident MDS assessments.
• Manage the care planning process from initial assessment to ongoing updates.
• Ensure timely and accurate completion of all assessments and documentation.
• Communicate effectively with physicians, therapists, and other healthcare providers regarding care plans.
• Maintain up-to-date understanding of state and federal long-term care regulations.
