Monarch Healthcare Management is seeking an MDS Coordinator-LPN to edit, lock, and transmit MDS assessments, ensuring timely, accurate completion and regulatory compliance. The role supports reimbursement through proper RAI processes and collaborates with the IDT to optimize resident care plans.

The ideal candidate is a Minnesota-licensed LPN with CPR certification, strong communication skills, and experience in nursing facilities.

Qualifications

• Must be a Licensed Practical Nurse (LPN) in Minnesota.
• Certification in CPR and emergency health care provider required.
• Ability to communicate verbally and in writing about the RAI process.
• Must perform essential functions with or without accommodation.
• Must qualify for employment after criminal background per Minnesota DHS.
• Healthcare experience in skilled nursing facility or assisted living preferred.
• Experience with MDS and RAI preferred.

Responsibilities

• Edit, lock, and transmit all MDS assessments.
• Ensure timely completion of the MDS with accuracy and regulatory compliance.

MDS Coordinator- Licensed Practical Nurses (LPN)

Position Summary:

The MDS Coordinator is accountable for editing, locking and the transmission process of all MDS assessments. The overall responsibility of this position is to help assure timely completion of the MDS with a focus on accuracy, case mix maximization, and regulatory compliance. He/she oversees the clinical and regulatory component of MDS completion related to reimbursement. This position also coordinates care plan completion with the Interdisciplinary Team (IDT), and renders clinical support to nursing staff to enhance accurate assessments, care plans and excellent quality of care.

Qualifications:

• Must be a Licensed Practical nurse (LPN) in the state of Minnesota




• Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.




• Must possess the ability to communicate verbally and in writing when directing the Resident Assessment Instrument (RAI) process and when performing and assessing care for residents.




• Must be able to perform essential functions of the position with or without reasonable accommodation.




• Must qualify for employment after criminal background study, per godliness of Minnesota Department of Humans services




• Experience in healthcare, skilled nursing facility or assisted living communitypreferred




• Experience with MDS and Resident Assessment Instrument (RAI)preferred
