Position Summary
The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessment process for residents in accordance with federal and state requirements. This position works closely with nursing, therapy, social services, dietary, activities, providers, and other members of the interdisciplinary team to ensure accurate assessments, appropriate care planning, timely documentation, and regulatory compliance.
The MDS Coordinator serves as a key resource for the interdisciplinary team and helps ensure that resident needs, services, and changes in condition are accurately reflected in the medical record and care plan.
Essential Duties and Responsibilities
- Coordinate and complete MDS assessments according to required schedules and regulatory guidelines.
- Review resident medical records, clinical documentation, nursing documentation, therapy records, and other available information to support accurate MDS completion.
- Conduct resident interviews and gather information from residents, families, staff, and other appropriate sources as needed.
- Coordinate with members of the interdisciplinary team to obtain accurate and complete information for MDS assessments.
- Identify changes in resident condition that may require additional assessment, documentation, or care plan intervention.
- Participate in and/or coordinate interdisciplinary care plan meetings.
- Develop, update, and maintain resident care plans based on assessment findings and identified resident needs.
- Monitor MDS schedules, assessment deadlines, and required documentation to ensure timely completion and submission.
- Review documentation for accuracy, consistency, and completeness and follow up with appropriate departments when clarification or additional documentation is needed.
- Collaborate with nursing, therapy, dietary, social services, activities, infection prevention, providers, and other departments regarding resident care and assessment information.
- Assist with ensuring that resident services and clinical conditions are accurately captured and supported by documentation.
- Maintain current knowledge of applicable CMS, federal, state, and facility requirements related to MDS, resident assessment, care planning, and documentation.
- Assist with survey readiness and respond to MDS-related requests for information.
- Maintain confidentiality of resident information in accordance with HIPAA and facility policies.
- Participate in quality assurance and performance improvement activities as assigned.
- Perform other duties as assigned by the Director of Nursing or Administrator.
Qualifications
- Current, active Maryland RN license.
- Previous experience in a skilled nursing or long-term care setting preferred.
- Previous MDS experience strongly preferred.
- Knowledge of the MDS/RAI process and long-term care regulatory requirements.
- Strong clinical assessment and documentation skills.
- Ability to organize and prioritize multiple assessments, deadlines, and responsibilities.
- Strong communication and interpersonal skills.
- Ability to work effectively with an interdisciplinary team.
- Proficiency with electronic medical records and standard computer applications.
Physical Requirements
- Ability to sit, stand, walk, and move throughout the facility as needed.
- Ability to communicate effectively with residents, families, staff, and interdisciplinary team members.
- Ability to perform duties in a skilled nursing facility environment.
- Ability to maintain regular attendance and meet assigned deadlines.
