Villa Healthcare in Michigan is seeking an experienced MDS Coordinator RN to manage the Resident Assessment Instrument (RAI) process, complete MDS assessments, and develop care plans in compliance with federal and state regulations. You will collaborate with an interdisciplinary team to ensure timely, accurate documentation that supports resident-centered care and Medicare/Medicaid reimbursement.

Qualifications include an active RN license in Michigan, CPR certification, and SNF/MDS experience;

Qualifications

• Coordinate MDS completion with state/federal requirements.
• Facilitate care planning with interdisciplinary team and families.
• Enter ICD-10 codes accurately for reimbursement.
• Educate staff on MDS documentation practices.
• Stay updated on long-term care regulations and MDS processes.
• Assist with QA/PIP initiatives.

Responsabilités

• Coordinate MDS in compliance with state/federal rules and timelines.
• Facilitate weekly interdisciplinary care conferences and Medicare meetings as assigned.
• Enter and correct ICD-10 codes per physician documentation and guidelines.
• Monitor resident condition changes and initiate required assessments.
• Review hospital records to accurately complete MDS scoring and billing.
• Support Triple Check process for Medicare claims alignment.
• Educate staff on MDS sections (ADLs, mood, function).
• Participate in QAPI initiatives and stay current on regulations.
• Perform other duties as assigned.
