Job summary

Avalon Care Center Yakima, part of Avalon Health Care Group, is hiring an MDS Coordinator (RN) for an on-site role in Yakima, WA. The position focuses on leading the MDS process, resident assessments, and regulatory compliance to support high-quality resident care.

Qualified candidates hold a WA RN license (or NLC-eligible) and have prior MDS Coordinator experience, with strong knowledge of RAI, care planning, and interdisciplinary collaboration. Salary is hourly and benefits are provided.

Qualifications

• Current Washington RN license (or NLC-eligible).
• Previous MDS Coordinator experience.
• Strong knowledge of the RAI process and case management.
• Strong attention to detail and organizational abilities.
• Ability to meet all employment screening requirements.

Responsibilities

• Oversee the MDS process and resident assessments
• Ensure accurate, timely MDS completion and regulatory compliance
• Coordinate care planning with interdisciplinary teams
• Support Medicare and Managed Care requirements
• Drive quality outcomes and reimbursement accuracy
• Serve as a key clinical and operational partner within the facility
