Job summary

Avalon Care Center Yakima in Yakima, WA seeks an MDS Coordinator to lead assessment accuracy and resident care. You will oversee the MDS process, drive quality outcomes, and collaborate with care teams to meet regulatory requirements.

• Ensure accurate, timely MDS completion and regulatory compliance.
• Coordinate care planning with interdisciplinary teams.
• Support Medicare and Managed Care requirements.
• Drive quality outcomes and reimbursement accuracy.
• Serve as a key clinical and operational partner within the facility.
