Job Title: MDS Coordinator, RN

Responsibilities

1. Ensure appropriate reimbursement for patient care services by participating in the Patient Assessment Process, including completion of MDS and Care Area Assessments (CAAs).
2. Develop individualized patient care plans, identify needs, and project outcomes in compliance with Federal and State regulations.
3. Contribute to the Resident Assessment Instrument (RAI) process, ensuring proper completion, encoding, and electronic transmission of MDS.
4. Identify changes in patient condition and coordinate assessments with the interdisciplinary team.
5. Assist with Activities of Daily Living training for new hires and provide ongoing education to staff.
6. Attend educational programs to stay current with RAI and Prospective Payment System regulations.

Qualifications

• Must be a licensed Registered Nurse in good standing.
• Previous experience in hospital, long-term care, or related healthcare facility required.
• Experience with completing MDSs and Care Plans is required.
• Knowledge of nursing practices, laws, and regulations governing long-term care.
