Job summary

Volare Health, LLC in Eugene, OR is seeking an MDS Coordinator (RN) to work onsite at the facility. The role pays $50-59.50/hr DOE and involves collaboration with the interdisciplinary team to assess residents' needs.

The MDS Nurse will participate in pre-admission information gathering, assist in ARD determination for PPS MDS assessments, and ensure timely completion of resident assessments per regulations. Strong RN licensure and familiarity with RAI are required.

Qualifications

• Active RN license in the state of practice.
• Familiar with RAI process and case management.
• Strong interpersonal communication skills.

Responsibilities

• Participates in the pre-admission process to obtain essential information for MDS optimization.
• Assists in review of resident care and ARD setting for PPS MDS assessments.
• Ensures accurate and timely completion of resident assessments per state and federal regulations.
• Completes accurate coding of the MDS from interviews, observations and records.
• Maintains the MDS schedules.
• Other duties as assigned.

Summary

The MDS Nurse works at the facility in collaboration with the interdisciplinary team to assess the needs of the residents.

Responsibilities

• Participates in the pre-admission process to ensure essential information needed for MDS optimization is obtained from the referral source(s).
• Assists in the review of resident care and the setting of the ARD to ensure the most appropriate ARD is utilized for Prospective Payment System MDS Assessments.
• Ensures accurate and timely completion of resident assessments according to state and federal regulations.
• Completes accurate coding of the MDS with information obtained from interviews with facility staff, residents, and family members, as well as observation of the resident and medical record review.
• Maintains the MDS schedules.
• Other duties and responsibilities as assigned or needed.

Qualifications

• Current, active Registered Nurse (RN) License in State of practice.
• Well-versed in inter-personal communications.
• Familiar with the RAI process and Case Management.
