Job summary

Graduate Post Acute is seeking an MDS Coordinator to ensure timely, accurate MDS assessments and compliant documentation. You will work with nursing and clinical teams to support resident care planning, regulatory compliance, and reimbursement processes.

The ideal candidate holds an active RN license with 3+ years in long-term care and MDS/RAI experience, applying strong clinical assessment skills and regulatory knowledge to improve care outcomes.

Qualifications

• RN license required with active status.
• Minimum of 3 years of clinical experience in long-term care.
• Prior MDS/RAI experience required and familiarity with Medicare/Medicaid.
• Strong clinical assessment skills with accuracy and attention to detail.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments in accordance with regulatory requirements and deadlines.
• Verify compliance with federal, state, and facility regulations related to MDS documentation and submission.
• Supervise MDS data entry, validation, and transmission processes for accuracy.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership team.
• Provide feedback to clinical teams and address documentation-related concerns.
• Assist with audits and regulatory reviews, providing required documentation.
• Support Medicare/Medicaid compliance and respond to inquiries.
• Stay current on changes to Medicare, Medicaid, and RAI/MDS regulations.
• Contribute to MDS-related quality improvement initiatives.
• Collaborate with interdisciplinary teams to accurately reflect resident care needs.
