Job summary

Brockton Post Acute and Healthcare Center is seeking an experienced MDS Coordinator to ensure timely, accurate, and compliant completion of all MDS assessments. You will supervise data entry and work with the DON and leadership to drive quality care and regulatory compliance.

The role requires an active RN license and at least 3 years in a long-term care setting, with prior MDS/RAI experience and strong attention to detail.

Qualifications

Responsibilities

• Ensure timely completion of all MDS assessments per regulations.
• Supervise MDS data entry, validation, and transmission.
• Identify and resolve data discrepancies and validation issues.
• Prepare MDS reports for the Director of Nursing and leadership.
• Stay current on Medicare/Medicaid guidelines and regulations.
• Collaborate with interdisciplinary teams on resident care needs.
