Job summary

Mt. Tabor Health & Rehab in Portland, OR is seeking a part-time MDS Coordinator (20 hours/week). You will serve as the clinical lead for resident assessments and care planning, coordinating with multiple departments for compliant documentation and improved outcomes.

The ideal candidate holds an active RN or LPN license in Oregon with MDS/RAI experience and PDPM knowledge. Certification in MDS/RAI is preferred or willingness to obtain, and collaboration with nursing, rehab and admin teams is

Qualifications

• Active RN or LPN license in Oregon.
• MDS/RAI knowledge and experience.
• Strong PDPM and CMS guideline understanding.
• Excellent attention to detail and organization.
• Ability to collaborate with nursing, rehab and admin teams.
• Certification in MDS/RAI preferred or willingness to obtain.

Responsibilities

• Complete and manage timely MDS assessments per RAI and CMS guidelines.
• Coordinate interdisciplinary care planning meetings.
• Review documentation to support coding accuracy for PDPM.
• Monitor quality measures and support QA initiatives.
• Educate staff on best practices in documentation and care planning.
• Stay current with regulatory updates and ensure compliance.
