Job summary

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing.
• Competence in MDS and RAI processes including completion, coding and regulatory compliance.
• Proficiency in care planning and updating individualized care plans.
• Experience or interest in geriatric nursing and long-term care regulations.
• Ability to interpret clinical documentation, analyze data, and maintain EHR.
• Strong communication and collaboration with interdisciplinary teams.
• Strong organizational skills and ability to manage multiple assessments and deadlines.
• Prior long-term care experience preferred; Medicare/Medicaid reimbursement knowledge is a plus.

Role Description

The MDS Coordinator is responsible for managing the Resident Assessment Instrument (RAI) process, ensuring accurate, timely completion of Minimum Data Set (MDS) assessments in compliance with regulatory requirements. This full-time, on-site role, located in the Portland, Oregon Metropolitan Area, involves coordinating interdisciplinary care planning, collaborating with nursing and therapy teams, and maintaining thorough documentation to support appropriate reimbursement and quality measures. Day-to-day responsibilities include reviewing clinical records, identifying changes in resident status, updating care plans, and entering data into the electronic health record system. The MDS Coordinator also educates staff on MDS-related processes, participates in quality improvement initiatives, and supports surveys and audits as needed.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing, with strong clinical assessment skills.
• Competence in MDS and RAI processes, including experience with MDS completion, coding, and regulatory compliance.
• Proficiency in Care Planning and Care Plans, with the ability to develop and update individualized care plans based on resident needs.
• Experience or strong interest in Geriatric Nursing, including understanding of long-term care regulations and best practices.
• Ability to interpret clinical documentation, analyze data, and maintain accurate electronic health records.
• Effective communication and collaboration skills for working with interdisciplinary teams and residents’ support networks.
• Strong organizational skills, attention to detail, and the ability to manage multiple assessments and deadlines.
• Prior long-term care or skilled nursing facility experience preferred; familiarity with Medicare/Medicaid reimbursement is an asset.
