Qualifications

• Valid Texas nursing license.
• CPR certification.
• Minimum of one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare program.
• Knowledge of long‑term care standards per Federal Regulations, Conditions of Participation and State regulations.

Responsibilities

• Lead the RAI process for each resident and ensure transmission and verification of receipt by the State.
• Oversee the MDS workflow to ensure accuracy and compliance with regulatory requirements.

We are searching for a Licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high-performing team and culture. The primary purpose is to complete the RAI process for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State.

Qualifications for MDS Coordinator:

• Valid Texas Nurse License.
