Job summary

The Center at Cordera is seeking a full-time LVN MDS Coordinator to develop and coordinate MDS, CAAs, and Care Plans in accordance with state and federal requirements. You will join the facility’s Interdisciplinary Team to ensure timely assessments and accurate documentation.

Ideal candidates have LVN licensure, at least two years in skilled or long-term care, and experience with the RAI/MDS process; MDS certification is preferred.

Qualifications

• LVN license in good standing.
• 2+ years in skilled or long-term care.
• Experience with RAI/MDS process; MDS certification preferred.
• Ability to coordinate IDT and complete MDS/CAAs and care plans.

Responsibilities

• Coordinate IDT to timely complete MDS and CAAs.
• Plan ARDs using MDS Scheduler.
• Communicate ARD selections to IDT.
• Complete MDS sections and CAAs as assigned.
• Complete Care Plan items and attend meetings.
• Monitor documentation to support MDS coding.
• Participate in skilled utilization and state case mix management.
• Teach MDS item completion to IDT and CNAs.
