Job summary

The Center at Cordera in Colorado Springs, CO is seeking a full-time LVN MDS Coordinator to develop and coordinate the MDS, CAAs, and Care Plan in accordance with State and Federal requirements.

You will work with the Interdisciplinary Team to ensure timely assessments, ARD scheduling, MDS completion, CAAs, and care planning, while mentoring CNAs and supporting nursing staff. This on-site role emphasizes accuracy, collaboration, and regulatory compliance.

Qualifications

• Two+ years of experience in skilled or long-term care.
• Experience with RAI process and MDS certification preferred.

Responsibilities

• Coordinates the interdisciplinary team to complete assessments on time.
• Plans MDS ARD dates using MDS Scheduler.
• Communicates ARD selections to the IDT.
• Completes MDS sections and CAAs as assigned.
• Maintains documentation to support MDS coding and participates in care planning.
