Paradigm at Creekside is seeking an MDS Coordinator (LVN/RN) to join our care team in Gahanna, OH. The role focuses on maintaining MDS status, coordinating required assessments, and ensuring accurate care plans.

A strong background in long-term care and familiarity with RAI processes are preferred. The ideal candidate will possess current licensure, demonstrated leadership, and proficiency with Point Click Care.

Qualifications

• Must hold an active LVN or RN license in the state.
• RN license preferred.
• 2 years' long-term care experience preferred.
• 6 months of rehabilitative/restorative nursing experience preferred.
• 2 years' experience in RAI process preferred.
• Ability to make independent decisions when warranted.
• Demonstrate leadership and organizational skills.
• Strong familiarity with Point Click Care preferred.
• Proficient with Outlook/Office/Internet.

Responsibilities

• Maintain the current MDS status of residents in accordance with laws and facility policies.
• Schedule OBRA, Medicare PPS, and Medicaid assessments and communicate schedules.
• Monitor clinical systems and coordinate MDS reassessments and care plans.
• Set ARD dates with the interdiscplinary team to reflect resident status.
• Complete MDS and CAAs accurately per guidelines.
• Attend daily Casemix, weekly Utilization Review, and monthly Triple Check meetings.
• Maintain active assessments for at least 15 months per Federal Regulations.
• Transmit MDS data electronically to the state as required.
• Adhere to facility policies governing nursing services.
• Participate in facility surveys and contribute to care planning.
