Paradigm at Creekside - Gahanna, OH is seeking an LVN/RN to join as MDS Coordinator. This full-time day position focuses on MDS accuracy, scheduling required assessments, and coordinating with the interdisciplinary team.

You will lead ARD planning, complete MDS sections, and participate in Casemix, Utilization Review, and Triple Check meetings. Proficiency with Point Click Care and MS Office is expected.

Qualifications

• Must possess an active Ohio LVN or RN license; RN license preferred.
• 2+ years in long-term care; rehab/restorative nursing experience preferred.
• 2+ years in RAI process completion preferred; strong leadership.

Responsibilities

• Maintain MDS status for each resident per federal/state laws and facility policies.
• Schedule OBRA, Medicare PPS, and/or Medicaid assessments; communicate schedules to departments and families.
• Monitor clinical changes; coordinate MDS reassessment and revised care plans.
• Set ARD dates with the IDT to reflect resident needs and services.
• Complete MDS/CAAs per policies; participate in Casemix, Utilization Review, and Triple Check meetings.
• Maintain active assessments for at least 15 months as required.
• Transmit MDS data electronically to state; ensure policy compliance.
• Provide resident/family education; respond to inquiries; CPR when needed.
