Job summary

Creekside Care Center in Ohio is seeking a motivated LVN/RN to join our team as MDS Coordinator. You will maintain MDS status, schedule required assessments, and coordinate with an interdisciplinary team to reflect resident needs and services.

The ideal candidate will have 2+ years in Long-Term Care, RAI process knowledge, a current Ohio nursing license (RN preferred), and familiarity with Point Click Care. Strong leadership and organizational skills are essential.

Qualifications

• Must hold a current, active license to practice as a Licensed Vocational Nurse or Registered Nurse in Ohio.
• Registered Nurse license preferred.
• Experience in Long-Term Care is preferred.
• Experience with the RAI process is preferred.
• Ability to lead, organize, and maintain a professional attitude.

Responsibilities

• Maintain the current MDS status of each resident in line with federal/state laws and facility policies.
• Schedule OBRA, Medicare PPS, and Medicaid assessments and communicate schedules to departments and families.
• Monitor clinical systems and coordinate MDS reassessment and care-plan updates.
• Set ARD in coordination with the interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs accurately per policies.
• Attend daily Casemix Meetings, weekly UR Meetings, and monthly Triple Check Meetings.
• Maintain assessments in the active record for at least 15 months.
• Transmit MDS data electronically to state authorities per regulations.
• Follow facility policies governing day-to-day nursing operations.
• Participate in facility surveys and inspections as needed.
• Contribute to developing and implementing a care plan to support wellbeing.
• Report unauthorized access attempts to the facility information system.
• Educate residents, families, and caregivers as directed by Administration and physicians.
• Provide CPR and emergency support per orders and directives.
