Park Manor Humble in Humble, TX is seeking an experienced MDS/PPS Coordinator (LVN/RN) to join our team. The role focuses on coordinating resident assessments, regulatory compliance, and data reporting to support high-quality care.

Ideal candidates have at least 2 years in long-term care, MDS/PPS experience, TILE familiarity, and a current RN or LVN license in Texas; experience with MDS 3.0 and Point Click Care is required.

Qualifications

• Must have 2 years of long-term care experience.
• MDS/PPS experience required.
• TILE experience is required and MDS 3.0 fluency.

Responsibilities

• Coordinate Resident Assessments: admissions, annuals, quarters, Medicare, and change-of-condition assessments with timely communication.
• Ensure Regulatory Compliance: adhere to Federal and State guidelines for MDS.
• Monitor Documentation: review MDS and care plans for accuracy and organization.
• Utilize MDS Software: maintain proficiency in software systems for data entry and reporting.
• Lead Interdisciplinary Meetings: coordinate Medicaid, ITTM, and care plan meetings.
• Update Care Plans: revise plans based on current assessments and resident status.
• Provide Quality Reporting: generate and share Quality Indicators with leadership.
• Submit MDS Data: ensure timely transmission of MDS assessments per standards.
• Stay Current: attend development sessions to keep up with industry standards.
• Oversee Ancillary Data: report ancillary services to corporate billing for processing.

• Extensive knowledge with Long-Term Care Medicaid, MDS and Point Click Care necessary

MDS COORDINATOR

In this role, you will:

• Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.


• Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).


• Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.


• Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting.


• Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.


• Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status.


• Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.


• Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards.


• Stay Current: Attend professional development sessions to keep up with industry standards and best practices.


• Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing.

Knowledge / Experience:

• Must have 2 years of long term care experience


• Must have MDS / TILE experience


• Must be fluent in MDS 3.0

Licensure / Certifications:

• Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
