Park Manor Humble in Humble, TX is seeking an experienced MDS/PPS Coordinator to join our team. The Coordinator oversees admissions and assessments, ensuring timely communication with nursing and other departments, and maintains compliance with MDS guidelines.

The ideal candidate has 2+ years in long-term care, MDS 3.0 proficiency, and MDS/PPS experience, with RN or LVN licensure in Texas. We offer competitive pay and benefits.

Qualifications

• Must have 2 years of long term care experience.
• Must have MDS / TILE experience.
• Must be fluent in MDS 3.0.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance with Federal and State MDS guidelines.
• Monitor MDS and care plan documentation for accuracy and thoroughness.
• Maintain expertise in MDS software and data entry/reporting.
• Lead interdisciplinary meetings for Medicaid, ITTM, and care plans.
• Update and revise care plans based on assessments and resident status.
• Generate and distribute Quality Indicators reports to leadership.
