Qualifications

• The Clinical Standard: Active LPN license in Pennsylvania.
• MDS/RAI Experience: Hands-on MDS/RAI process knowledge.
• Certification: RAC-CT or LPNAC is a plus, strong clinical coding track record.
• Meticulous Attention to Detail: Analytical mindset, data integrity, problem-solving.
• Great Communication: Strong organizational skills, cross-department collaboration.

Responsibilities

• Master the MDS process: coordinate timely, accurate MDS assessments (OBRA and PPS) from admission to discharge.
• Collaborate with IDT: work with nursing, therapy, dietary, and social services to build person-centered care plans.
• Maximize clinical reimbursement: review documentation to support care and capture resource utilization data.
• Audit & support: conduct quality reviews of medical records and provide feedback to staff on documentation.
• Manage transmission data: assist in error-free electronic submission of MDS data to CMS.

WHAT YOU’LL DO (Your Impact):

• Master the MDS Process: Coordinate and oversee the timely completion of all MDS assessments (OBRA and PPS) from admission to discharge, ensuring 100% accuracy and compliance with Pennsylvania state and federal guidelines.

• Collaborate with the Interdisciplinary Team (IDT): Work closely with nursing, therapy, dietary, and social services to build truly person-centered care plans across our campus that accurately reflect resident needs.

• Maximize Clinical Reimbursement: Review clinical documentation with a sharp eye to ensure it fully supports the care provided, capturing the appropriate resource utilization data for accurate reimbursement.

• Audit & Support: Conduct routine quality reviews on medical records and provide helpful feedback to floor staff regarding the vital importance of complete, precise clinical documentation.

• Manage Transmission Data: Assist in ensuring the smooth, error-free electronic submission of all MDS data to CMS, executing course corrections whenever needed.

WHAT YOU BRING TO THE TEAM:

• The Clinical Standard: An active, unencumbered Licensed Practical Nurse (LPN) license in the state of Pennsylvania. (Must be an LPN to apply).

• MDS/RAI Experience: Hands-on experience managing or assisting with the MDS / RAI process in a skilled nursing or long-term care setting.

• Certification: RAC-CT (Resident Assessment Coordinator Certified) designation or LPNAC equivalent is a massive plus, but a proven track record of clinical coding excellence is highly valued!

• Meticulous Attention to Detail: An analytical mindset with a passion for compliance, data integrity, and complex problem-solving.

• Great Communication: Exceptional organizational skills and the ability to collaborate smoothly across different departments.
