Job summary

Lynn Care Center in Virginia is seeking an experienced MDS Coordinator (LPN or RN) to join our team. The role focuses on coordinating MDS/RAI processes to ensure accurate, timely assessments and regulatory compliance in a skilled nursing setting.

The ideal candidate will have a Virginia license and at least 1 year of MDS experience, with strong knowledge of MDS 3.0, RAI, and PDPM, complemented by excellent documentation and communication skills and the ability to work with an interdisciplinary

Qualifications

• Current, active LPN or RN license in Virginia.
• Minimum 1 year of MDS experience.
• Experience in skilled nursing or long-term care preferred.
• Strong knowledge of MDS 3.0, RAI, Medicare, Medicaid, and PDPM.
• Excellent assessment, documentation, and communication skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Complete accurate and timely MDS assessments in accordance with federal and state regulations.
• Coordinate the Resident Assessment Instrument (RAI) process.
• Ensure compliance with CMS guidelines and reimbursement requirements.
• Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans.
• Monitor assessment schedules and ensure documentation deadlines are met.
• Review medical records for accuracy and completeness.
• Participate in quality assurance and performance improvement initiatives.
• Stay current with regulatory changes affecting MDS and reimbursement.
