Job summary

Lynn Care Center in Virginia seeks an experienced MDS Coordinator (LPN or RN) to oversee the RAI/MDS process, ensuring accurate, timely assessments and regulatory compliance. You will collaborate with an interdisciplinary team to support quality resident care and proper reimbursement.

The role requires an active Virginia LPN or RN license and at least 1 year of MDS experience, preferably in a skilled nursing setting. Competitive pay and comprehensive benefits are offered.

Qualifications

• Current, active LPN or RN license in Virginia.
• Minimum 1 year of MDS experience.
• Experience in a skilled nursing or long-term care setting preferred.
• Strong knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM.
• Excellent assessment, documentation, and communication skills.
• Ability to work collaboratively with an interdisciplinary team.

Responsibilities

• Complete accurate and timely MDS assessments in accordance with federal and state regulations.
• Coordinate the Resident Assessment Instrument (RAI) process.
• Ensure compliance with CMS guidelines and reimbursement requirements.
• Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans.
• Monitor assessment schedules and ensure documentation deadlines are met.
• Review medical records for accuracy and completeness.
• Participate in quality assurance and performance improvement initiatives.
• Stay current with regulatory changes affecting MDS and reimbursement.
