Belmont Health and Rehabilitation in Columbus, IN is seeking a licensed LPN or RN to join our team as an MDS Coordinator. You will handle MDS scheduling and completion in line with OBRA/PPS guidelines and work with the interdisciplinary team.

Qualifications

• Requires license in Indiana (LPN or RN).
• Strong organization and attention to detail are essential.
• Excellent assessment and communication abilities required.

Responsibilities

• MDS scheduling and completion for OBRA/PPS/Managed Care using RAI guidelines.
• Assist with care plan scheduling and completion with the interdisciplinary team.
• Assign and update ICD-10 codes based on physician diagnoses on admission and with each MDS.
• Transmit OBRA/PPS MDS Assessments to CMS per Federal Guidelines.
• Oversee MDS documentation and charting requirements to support services.
• Educate staff on MDS processes as needed.
• Assist with data collection for audits (State, Medicare, Managed Care).
• Participate in facility and company meetings per policy.
• Become proficient with RAI rules/regulations and QM monitoring.
