Job summary

North Olmstead Nursing & Rehabilitation in Ohio is seeking a caring RN/LPN to join our team as the MDS Coordinator. The MDS Coordinator plays a vital role in the nursing administrative team and ensures quality resident assessments.

The role involves coordinating the MDS process according to Federal and State guidelines, assembling data from assessments, and communicating with the interdisciplinary team to meet deadlines.

Qualifications

• Must hold an active RN or LPN license.
• Experience coordinating MDS processes.
• Knowledge of Federal and State regulations related to MDS.

Responsibilities

• Conduct and coordinate the development and completion of the resident assessment in accordance with Federal and State regulations as well as Company policy and procedure.
• Oversee and coordinate the development and completion of the resident assessment, including the implementation of RAPS and Triggers.
• Assemble info from the Initial Nursing Assessment, resident interview, and clinical record review to complete the nursing portion of the MDS within 10-14 days of admission or annual review, and when there is a significant change in resident condition.
• Notify all members of the interdisciplinary team at least one week in advance of the MDS due date for all new admissions, annual reviews, and significant changes in resident condition.
• Monitor and follow up with team members as needed to verify that all assigned sections of the MDS are completed, dated and signed within designated time frames.
