Job summary

The Boulevard Post Acute is seeking an experienced MDS Coordinator for a full-time, on-site role in Virginia. You will ensure timely, accurate MDS assessments and maintain compliance with regulatory requirements to support resident care planning and reimbursement.

Qualified candidates will have an RN/BSN/LPN license, 3+ years in long-term care, and demonstrated MDS/RAI experience. Join a team focused on accuracy, quality, and regulatory success in a supportive environment.

Qualifications

• RN/BSN/LPN license required.
• Minimum of three (3) years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail and accuracy.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments.
• Verify compliance with federal, state, and facility regulations.
• Supervise MDS data entry, validation, and transmission processes.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership team.
• Provide feedback to clinical teams and address operational concerns.
• Participate in surveys, audits, and regulatory reviews.
