Job summary

Cordova Wellness & Rehabilitation Center is seeking a dedicated MDS Coordinator (LPN or RN) in Cordova, Tennessee. This full-time position offers a supportive environment where you can join our compassionate care team.

Your responsibilities include coordinating the Resident Assessment Instrument process and ensuring compliance with state and federal guidelines. Qualifications

• 2-5 years experience with PDPM, CMI, and Quality Measures.
• RAC Certification preferred.

Responsibilities

• Attend weekly education meetings on MDS changes.
• Coordinate the facility’s resident assessment process.
• Complete all MDS assessments and supporting documentation.
• Collaborate with the interdisciplinary team.
• Maintain confidentiality of protected health information.
