Avalon Care Center at Northpointe in Spokane, WA is hiring a MDS Coordinator (LPN). This full-time role focuses on coordinating and completing MDS assessments, overseeing OBRA, Medicare and care planning, and ensuring timely regulatory compliance.

You will work with an interdisciplinary team, bring your Washington LPN license (or NLC eligible), and leverage your MDS experience to improve resident outcomes. A $5,000 sign-on bonus is offered, and compensation is tied to experience.

Qualifications

• Current LPN license in Washington State or NLC eligible.
• Previous MDS and skilled nursing facility experience required.
• Knowledge of the RAI process and case management.
• Strong communication and organizational skills.
• Ability to work effectively within a team.
• Ability to pass employment screenings.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time
• Oversee Medicare, Managed Care, and OBRA assessment processes
• Support care planning and interdisciplinary team collaboration
• Help ensure regulatory compliance and reimbursement accuracy
• Monitor resident assessments from admission through discharge
• Promote quality outcomes and resident-centered care
