Center at Cordera in Colorado Springs, CO is seeking an experienced MDS Coordinator to lead the MDS/RAI process within the IDT. You will plan ARDs, complete MDS sections, CAAs, and Care Plans, while ensuring accurate documentation for coding and compliance.

Teaching and mentoring CNA staff on ADL documentation are part of the role. Two years in a skilled or long-term care setting with RAI/MDS experience is preferred, along with an active RN license.

Qualifications

• Current RN License.
• At least two years’ experience in a skilled or long term care setting.
• Experience with RAI process and MDS certification preferred.

Responsibilities

• Coordinates the interdisciplinary team in timely completion of the assessments.
• Utilizes MDS Scheduler to plan and set MDS ARDs.
• Communicates ARD selections for specific MDSs to IDT.
• Completes the MDS sections as assigned.
• Completes CAAs as required.
• Completes Care Plan items and attends Care Plan meetings.
• Monitors documentation to support MDS coding.
• Mentors CNA staff and provides teaching related to ADL documentation.
• Performs nursing duties as assigned.
