Qualifications

• Must possess an LPN/RN nursing degree with active state license.
• Experience coordinating resident assessments (MDS) and care plans.
• Ability to work with interdisciplinary teams and maintain compliance.

Responsibilities

• Conduct and coordinate development and completion of resident assessments (MDS).
• Maintain and update policies governing MDS and care plans.
• Oversee PPS scheduling, care plan meetings, and interdisciplinary reviews.
• Assist with discharge planning and collaboration with social services.
