Job summary

Responsibilities include reviewing MDS data, conducting assessments, developing restorative programs, and coordinating Medicare-related admissions and discharges.

Qualifications

• Active IL Nursing licensure preferred with MDS and rehabilitation credentials.
• 2–3 years’ applicable experience as LPN/RN and 1–3 years’ experience in MDS; preferred in long-term care.
• Ability to maintain a high level of confidentiality.
• Energetic, professional with exceptional interpersonal skills.
• Excellent communication and customer service skills.

A skilled nursing home facility, located in Chicago, IL is currently seek an experienced LPN/ RN MDS/Care Plan Coordinator . In this role, the MDS Coordinator is responsible for planning, organizing, developing and directing the MDS assessments and care plans in accordance with current applicable federal, state and facility standards, guidelines and regulations. This position is responsible for appropriate reimbursement through accurate completion off the MDS. This person will also develop, implement and supervise the restorative nursing program and coordinates the corresponding programs.
