Chicago Skilled Nursing in Chicago, IL is seeking an experienced MDS Coordinator to join our team at our skilled nursing facility. You will develop, monitor, and modify care plans, perform resident assessments, and assist in the discharge process to ensure high-quality care.

Applicants must hold an Illinois RN license, CPR certification, and have at least two years of experience as an MDS Coordinator, plus two years as an RN in an SNF and one year as a CNA.

Qualifications

• Must hold unencumbered Illinois RN license.
• Graduate from an accredited nursing program.
• CPR Certification required.
• Minimum two (2) years experience as an MDS Coordinator.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in discharge planning.
• Ensure high standard of care and compliance with regulatory requirements.
