Job summary

The Laurels of Bon Air is seeking an experienced MDS Coordinator to oversee the RAI process and coordinate care plans for residents. The role supervises the Care Management Nurse and MDS Nurse, requiring extensive knowledge of MDS, PPS, and Medicare timelines.

Ideal candidates will have RN or LPN licensure, experience as an MDS Nurse, and strong interdisciplinary collaboration skills to ensure high-quality resident care across services within the Richmond, VA area.

Qualifications

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Maintains PPS board for monitoring Medicare days and RUGs utilization in the absence of the Care Management Coordinator.

Responsibilities
