Job summary

The Laurels of Bon Air in Virginia is seeking an experienced MDS Coordinator to oversee the RAI process and coordinate care planning for residents. The role supervises the Care Management Nurse and MDS Nurse, ensuring timely assessments and compliant documentation.

Ideal candidates will have RN or LPN licensure and proven MDS experience, with familiarity in PPS and interdisciplinary coordination. Comprehensive benefits and a supportive team culture are offered.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure.
• Knowledge of the Resident Assessment Instrument (RAI) process; PPS experience preferred.
• Experience as an MDS Nurse.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with caregivers, physicians, nursing, social services, therapy, dietary and activity staff.
• Reviews hospital, consults and outside information for assessments and care plans.
• Coordinates, identifies, and initiates significant changes to MDS.”
• Conducts PPS meetings and maintains PPS board for Medicare days and RUGs in the absence of the Care Management Coordinator.
• Stays current with AANAC requirements.
