Job summary

The Laurels of Salisbury is seeking an experienced Registered Nurse to join our MDS/Care Management team. This role focuses on remaining clinically involved in patient care while maintaining autonomy in workload and schedule.

Ideal candidates have MDS nurse experience, are familiar with RAI and PPS processes, and can coordinate care plans across care disciplines. We offer a robust benefits package and supportive team environment.

Qualifications

• Registered Nurse (RN) with active licensure.
• Knowledge of the Resident Assessment Instrument (RAI) and PPS concepts.
• Experience as an MDS Nurse in a skilled nursing setting.

Responsibilities

• Works the RAI process and conducts resident assessments and care plan coordination.
• Completes MDS, CAA’s and care plans within regulated time frames.
• Assesses residents through physical exams, interviews and chart reviews.
• Discusses resident care needs with the interdisciplinary care team.
• Reviews information from hospital and outside agencies to support assessments and care planning.
• Coordinates and initiates significant changes to MDS when needed.
• Prepares PPS meetings and maintains PPS board for Medicare days and RUGs.
• Stays current with AANAC requirements.
