PACS Group is seeking a dedicated MDS Nurse (LVN/LPN) to coordinate and complete MDS assessments, ensuring accurate documentation and compliance with CMS and state regulations. You will work with interdisciplinary teams to promote high-quality resident care and accurate reimbursement.

The role requires an active PA LVN/LPN license, CPR certification, and the ability to manage documentation and education for staff, with a focus on patient rights and safety.

Qualifications

• Current, unencumbered PA nursing license and good standing.
• Active CPR certification and continuing education obligations.
• Ability to read, write, and communicate in English clearly.
• Must pass background check and health requirements.

Responsibilities

• Complete MDS assessments and CAAs within required timelines.
• Ensure accurate clinical documentation to support care planning.
• Maintain CMS/state regulatory compliance for Medicare/Medicaid.
• Collaborate with nursing, therapy, social services, and dietary teams.
• Provide education on documentation standards to staff.
