Job summary

Topaz Fiscal Services is seeking an LPN with MDS experience to oversee the MDS process from admission to discharge. You will coordinate care management meetings and ensure compliance with regulations across the facility.

Must hold a state LPN license, have at least two years in MDS, and obtain RAC-CT certification within 90 days of hire. Strong data entry and documentation skills are essential for accurate coding and reporting.

Qualifications

• LPN licensure within the state and at least two years of MDS experience in long-term care.
• Current AANAC RAC-CT certification or obtain within 90 days of hire.
• Knowledge of MDS processes from admission to discharge and coordination with facility staff.

Responsibilities

• Collaborate with facility team to assess resident needs.
• Coordinate and manage care management meetings.
• Ensure compliance with federal and state MDS regulations.
• Monitor MDS and care plan documentation for all residents.
• Maintain MDS status per guidelines and track schedules.
• Enter resident data into computerized programs accurately.
• Code MDS using data from records and interviews.
• Attend meetings to gather information and update records.
• Participate in quality assurance activities as required.
• Submit required documentation electronically per policy.
