Job summary

Premier Living and Healthcare Center in Lake Waccamaw, NC seeks an MDS Coordinator to ensure accurate MDS data and regulatory compliance. You will manage data in MDS software, coordinate concurrent and comprehensive assessments, and develop individualized care plans.

Collaborating with healthcare professionals, you’ll ensure accuracy and HIPAA adherence, participate in quality initiatives, and support staff development while upholding a patient-centered care philosophy.

Qualifications

• Bachelor's degree in Nursing or related field (or equivalent experience).
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment with multiple priorities.
• Detail-oriented and organized with excellent analytical skills.

Responsibilities

• Collect and maintain accurate and thorough data in our Minimum Data Set (MDS) software
• Facilitate the MDS process, including conducting concurrent and comprehensive assessments
• Develop and implement individualized care plans for patients
• Collaborate with healthcare professionals to ensure accuracy and completeness of MDS data
• Maintain confidentiality and adhere to HIPAA guidelines
• Participate in quality improvement initiatives and staff development programs
• Demonstrate a commitment to patient-centered care and a passion for improving the healthcare experience
